Thank you for scheduling your appointment with us! In order to prevent the spread of COVID-19, please follow these important instructions when you arrive to your appointment:

• Please remain in your car and call the office when you arrive in the parking lot. Our waiting room is closed.

• One of our team members will guide you through entering the office which will include a health screening questionnaire and a touchless temperature check.

• If you have had any contact with a COVID-19 positive person, have been tested for COVID-19, or have any symptoms of COVID-19 (including a fever of 100.4 F) please notify us.

• Please notify us if you can not come to your appointment or think you will be late. Because of the strict infection control protocols we adhere to, we cannot accommodate late arrivals in respect to other appointments. We are happy to reschedule you to a better time if need be. Thank you.
